Who Should Consider a Breast MRI?

Breast Cancer Screening
Breast MRI is recommended if you:
- Have risk factors for breast cancer.
- Have had a suspicious mammogram and/or breast ultrasound.
- Are referred by a physician for further imaging.
High-Risk Screening Recommendations
Yearly mammograms combined with Breast MRI screenings are typically advised for women who are at higher-than-average risk of developing breast cancer. According to the American Cancer Society (ACS), high-risk women (greater than 20% lifetime risk) should start combined screenings at age 30. High-risk women include:
- Those with a BRCA1 or BRCA2 gene mutation.
- Those with a first-degree relative with a BRCA1 or BRCA2 gene mutation but who have not been tested themselves.
- Women with a lifetime risk of 20-25% or greater based on risk assessment tools focused on family history.
- Those who had radiation therapy to the chest for another type of cancer between the ages of 10 and 30.
- Individuals with genetic conditions such as Li-Fraumeni syndrome, Cowden syndrome, or Bannayan-Riley-Ruvalcaba syndrome.
Moderate-Risk Screening Recommendations
Moderate-risk women (15-20% lifetime risk) should discuss adding a Breast MRI to their yearly mammograms with their doctors. This includes:
- Women with a lifetime risk of 15-20%, according to family-history-based risk tools.
- Those with a personal history of breast cancer, ductal carcinoma in situ (DCIS), lobular carcinoma in situ (LCIS), or abnormal breast cell changes like atypical ductal hyperplasia.
- Women with extremely dense or unevenly dense breasts when viewed by mammograms.
Breast Implant Rupture Assessment
A Breast MRI is also the most sensitive test for detecting implant ruptures. If a rupture is suspected, your physician may refer you for an MRI to confirm the diagnosis and guide your next steps. Breast implant ruptures can occur as:
- Intra-capsular ruptures – contained within the capsule.
- Extra-capsular ruptures – silicone freely extravasates beyond the capsule.
